Wellness Nurse

The Wellness Nurse role at Franciscan Ministries supports residents at Franciscan Village, a senior living community in Lemont, IL. This exempt position provides clinical oversight, health assessments, and care coordination for individuals in an assisted living environment. The schedule follows standard community operations with no travel requirements.

Responsibilities

• Conduct comprehensive health assessments for prospective and current residents.
• Develop, implement, and update individualized service plans based on resident needs and physician recommendations.
• Monitor residents for changes in physical, cognitive, and emotional health and initiate appropriate interventions.
• Perform nursing assessments following falls, illness, hospitalization, or significant changes in condition.
• Coordinate physician appointments, laboratory services, therapy, hospice, home health, and other healthcare services.
• Maintain accurate, timely, and confidential clinical documentation.
• Provide clinical guidance and support to clinical associates at the community.
• Respond to resident emergencies and provide appropriate nursing interventions.
• Ensure compliance with resident rights, confidentiality, and professional nursing standards.
• Communicate resident health concerns and changes in condition to physicians, families, and responsible parties in a timely manner.
• Participate in resident care conferences and interdisciplinary meetings.
• Educate residents and families regarding health conditions, medications, and available healthcare services.
• Maintain compliance with all federal, state, and local assisted living regulations.
• Assist during state surveys and regulatory inspections.
• Ensure required nursing documentation is complete, accurate, and survey-ready.
• Maintain resident records in accordance with HIPAA and community policies.
• Perform other duties as assigned.
• Comply with all policies and procedures and any updates.

Requirements

• Graduate of an accredited nursing program.
• Current Licensed Practical Nurse (LPN) or Registered Nurse (RN) license.
• 1+ years of experience in a clinical/nursing setting within the senior living industry.
• 1+ years of supervisory experience preferred.

Benefits

• Salary range: USD $81,000.00 to $85,000.00 per year.
