Licensed Practical Nurse (LPN) - FT 1st shift & FT NOC with rotating weekends (8hr shifts)

This full-time Licensed Practical Nurse (LPN) role at St. Joseph Village, operated by Franciscan Ministries in Chicago, IL, covers 8-hour shifts on 1st shift and NOC schedules with rotating weekends. The position delivers direct nursing care to residents in a faith-based community setting. Responsibilities

• Direct daily activities of certified nursing assistants and resident care associates according to federal, state, and local regulations plus established policies.
• Apply the nursing model to deliver holistic resident care.
• Verify that assigned nursing personnel follow all policies and procedures.
• Handle resident admissions, discharges, and transfers as needed.
• Monitor resident status changes, notify the physician and family or representative, and document findings.
• Transcribe physician orders into the medical record and execute them as written.
• Prepare and administer medications per physician orders while monitoring for adverse effects.
• Perform wound treatments per physician orders, observe changes, and document results.
• Arrange follow-up appointments and transportation for residents.
• Provide nursing leadership to assigned personnel on the unit.
• Deliver direct care including colostomy changes, tube feedings, wound care, suctioning, and IV administration per current policies.
• Conduct rounds to confirm resident needs are met and staff complete assigned duties.
• Coordinate with the interdisciplinary team to address resident needs.
• Report incidents or unusual occurrences to the leadership team.
• Maintain adequate stock of medications, supplies, and equipment and notify appropriate staff of shortages.
• Initiate, review, and update care plans as required.
• Participate in inspection surveys and QAPI activities when assigned.
• Apply infection control principles and correct any identified practice deficiencies.
• Perform additional duties as assigned while complying with all policies and updates.

Requirements

• Nursing degree from an accredited college or university
• Current unrestricted LPN license in the practicing state
• 1+ years of nursing experience preferred
