Social Worker

The Social Worker role at Franciscan Ministries is a full-time exempt position based in Lemont, IL. The position supports the emotional and psychosocial needs of residents in a senior care community that includes skilled nursing services. The annual salary range is $55,000 to $61,000.

Responsibilities

• Perform assessments to determine residents' emotional, social, and behavioral requirements.
• Create individualized support plans together with residents, families, and the care team.
• Deliver short-term counseling, emotional support, and crisis intervention services to residents and families.
• Help residents and families adapt to transitions such as move-in, health changes, hospitalization, rehabilitation, and end-of-life care.
• Lead family meetings focused on care planning and addressing concerns.
• Arrange referrals to community resources, behavioral health providers, hospice, support groups, and government programs.
• Advocate for resident rights, autonomy, and person-centered care approaches.
• Support discharge planning and transition coordination as needed.
• Keep accurate, timely, and confidential records according to policies and regulations.
• Join interdisciplinary care conferences and work with nursing, rehabilitation, activities, dining, and other departments.
• Track changes in residents' emotional or psychosocial status and report concerns to the appropriate team members.
• Educate residents, families, and staff on topics including aging, dementia, grief, and available resources.
• Organize resident support groups and educational programs when appropriate.
• Assist with advance care planning discussions and provide information on advance directives upon request.
• Identify residents who may need mental health services and arrange suitable referrals.
• Contribute to a welcoming, respectful, and inclusive environment.
• Take part in quality improvement efforts and follow federal, state, and local regulations.
• Carry out additional duties as assigned and follow all policies and procedures.

Requirements

• Bachelor’s degree in social work required; master’s degree in social work preferred.
• Current unrestricted Licensed Clinical Social Worker (LCSW) license in the state of employment.
• Minimum 2 years of clinical social work experience.
• Minimum 2 years of experience in a skilled nursing home setting.
• Occasional travel required for community outreach or training events.
