Licensed Practical Nurse (LPN)*Assisted Living

This Licensed Practical Nurse (LPN) position is with Franciscan Ministries in an assisted living community located in Wheeling, IL. The role focuses on direct nursing care for residents along with consultation on health-related matters in the community. Daily oversight of nursing assistants and adherence to regulatory standards form core aspects of the work.

Responsibilities

• Direct daily activities of certified nursing assistants and resident care associates while following federal, state, and local regulations plus established policies.
• Apply the nursing model to deliver holistic care to residents.
• Verify compliance with policies and procedures among assigned nursing personnel.
• Handle resident admissions, discharges, and transfers as needed.
• Monitor changes in resident status, notify physicians and family representatives, and document findings.
• Transcribe physician orders into medical records and execute them as written.
• Prepare and administer medications according to physician orders while observing for adverse effects.
• Perform wound treatments per physician orders, observe changes, and document results.
• Arrange follow-up appointments and transportation for residents.
• Provide leadership to nursing personnel on the assigned unit.
• Deliver direct care including colostomy changes, tube feedings, wound care, suctioning, and IV administration per current policies.
• Conduct rounds to confirm resident needs are met and staff complete assigned duties.
• Collaborate with the interdisciplinary team to address resident needs.
• Report incidents or unusual occurrences to community leadership.
• Maintain adequate stock of medications, supplies, and equipment and notify appropriate staff of shortages.
• Initiate, review, and update care plans as required.
• Participate in survey processes during inspections.
• Engage in QAPI activities when assigned.
• Apply infection control principles and address any practice deficiencies.
• Complete additional duties as assigned while complying with all policies and updates.

Requirements

• Nursing degree from an accredited college or university.
• Current unrestricted Licensed Practical Nurse (LPN) license in the practicing state.
• One or more years of nursing experience preferred.

Benefits

• Hourly pay range of $35.00 to $38.00.
