Licensed Practical Nurse- Assisted Living Memory Support

This full-time Licensed Practical Nurse role is with Franciscan Ministries at Ancora Memory Support, a memory care assisted living community within the Mount Alverna Village continuing care retirement community in Parma, Ohio. The position covers the 7p-7a shift and focuses on person-centered dementia care in a setting that includes independent living, assisted living, skilled nursing, and an onsite rehabilitation clinic offering physical, occupational, and speech therapies.

Responsibilities

• Administer, control, and secure medications while managing reorders as needed.
• Carry out clinical nursing procedures required for resident care.
• Document all nursing care activities accurately.
• Provide assistance with activities of daily living when residents require support.
• Direct Resident Care Assistants and additional staff to address resident needs.
• Take part in restorative nursing initiatives and quality improvement efforts.
• Help orient new staff members and support new residents during move-in.

Requirements

• Graduate of an accredited LPN program
• Current Ohio LPN license
• Minimum of 2 years acute care nursing experience and 1 year in assisted living
