Ancora Memory Care Coodinator

The Ancora Memory Care Coordinator role at Franciscan Ministries supports residents in the memory care assisted living unit in Wheeling, IL. The position provides oversight for quality of care, regulatory compliance, and participation in the QAPI Committee while managing clinical staffing and resident service plans. The salary range for this full-time position is $80,000 to $90,000 per year.

Responsibilities

• Assist with interviewing, hiring, and orientation of new clinical associates.
• Monitor and support staffing schedules to maintain appropriate coverage.
• Conduct pre-admission, admission, and ongoing resident assessments and update Resident Service Plans.
• Participate in care conferences and maintain communication with families and residents.
• Perform routine rounds on the Ancora Unit to verify compliance and resident care standards.
• Maintain an attractive, safe, and sanitary environment for residents, staff, families, and vendors.
• Serve on the QAPI Committee.
• Audit and maintain electronic health records.
• Act as a member of the administrative staff with on-call responsibilities.
• Perform additional duties as assigned and follow all policies and procedures.

Requirements

• Current CPR certification.
• Minimum 2 years of clinical experience or leadership in assisted living or memory support settings.
• Proficiency with electronic health records, PointClickCare, and Microsoft applications preferred.
• Knowledge of nursing practices, medical procedures, and federal and state long-term care regulations.
