Memory Care Certified Nursing Assistant (CNA)- PM shift

The Memory Care Certified Nursing Assistant (CNA) role on the PM shift is open at Franciscan Ministries in Homer Glen, IL. This position supports residents in a memory care community through assistance with daily living tasks and monitoring of physical and cognitive status. The hourly pay range is $18.00 to $20.00.

Responsibilities

• Assist residents with activities of daily living such as bathing, dressing, grooming, toileting, and medication reminders according to individual care plans and community procedures.
• Perform lifting, turning, transferring, positioning, and transporting of residents between beds, chairs, bathtubs, wheelchairs, and lifts.
• Support dining services at assigned mealtimes including setup, cleanup, tray delivery, feeding assistance, and documentation of meal intake.
• Monitor residents' physical and cognitive wellbeing and report changes to nursing staff and supervisors.
• Deliver nutritional supplements at scheduled times, assist residents as needed, and document intake per community policy.
• Participate in life enrichment programming and provide meaningful activities for residents.
• Document resident care in a timely and accurate manner.
• Complete other duties as assigned and comply with all policies and procedures.

Requirements

• High School Diploma or GED
• Certified Nursing Assistant certification as required by state regulations
• CPR certification
• 1+ years of paid or volunteer experience working with seniors (preferred)
