Nurses

University Place, a continuing care retirement community operated by Franciscan Ministries, seeks Registered Nurses (RN) and Licensed Practical Nurses (LPN) for its skilled nursing and long-term care units in West Lafayette, Indiana. The onsite rehabilitation clinic provides inpatient and outpatient physical, occupational, and speech therapies. Positions are available full-time or part-time with every other weekend and rotating holidays required.

Responsibilities

• Direct daily activities of certified nursing assistants and resident care associates per federal, state, and local regulations and facility policies.
• Provide direct nursing care including colostomy changes, tube feedings, wound care, suctioning, and IV administration.
• Assess changes in resident status, notify physicians and families, and document accordingly.
• Transcribe and carry out physician orders; prepare and administer medications while monitoring for adverse effects.
• Perform wound treatments per physician orders and document changes.
• Schedule follow-up appointments and coordinate resident transportation.
• Conduct rounds to verify resident needs and staff performance; report incidents to leadership.
• Maintain adequate stock of medications, supplies, and equipment.
• Initiate, review, and update care plans; participate in surveys and QAPI activities as assigned.
• Ensure infection control principles are followed and report allegations of abuse, neglect, or exploitation.

Requirements

• Nursing degree from an accredited college or university.
• Current unrestricted RN license in the practicing state (LPN licensure also accepted per overview).
• 1+ years of clinical nursing experience, preferably in geriatrics.
• Ability to work full-time or part-time schedules including every other weekend and rotating holidays.
