Social Worker Sign On Bonus SNF

This full-time onsite Social Worker role is located at Vi at Bentley Village, a skilled nursing facility in Naples, Florida. The position includes a $4,000 sign-on bonus and centers on resident care within the SNF setting. The address is 2315 Leisure Lane, Naples, FL 34110.

Responsibilities

• Assess residents' psychosocial needs
• Coordinate services for residents
• Facilitate care planning
• Provide emotional support to residents and families
• Collaborate with the interdisciplinary team to promote resident well-being
• Support successful transitions of care
• Ensure compliance with regulatory requirements

Requirements

• 2 years of related experience, including at least 1 year in a health care setting such as a hospital, clinic, or rehabilitation center
• Bachelor's Degree in Social Work, Psychology, Gerontology, or Sociology from an accredited institution
• Proficiency in MS Office
• Compliance with Florida’s Care Provider Background Screening process
