Nurse Practitioner (NP) Primary Care

Nurse Practitioner (NP) Primary Care position with HarmonyCares, a national value-based provider of in-home primary care services for patients with complex healthcare needs. The role operates within an integrated, team-based, physician-driven model and is located in League City, Texas. Providers deliver care in patients' homes on a Monday through Friday schedule with minimal call responsibilities and no hospital rounds or holidays.

Responsibilities

• Provide primary care to high acuity patients in their homes Monday through Friday
• Focus on patient care rather than volume, allocating time to address individual patient needs
• Practice in home health, geriatrics, hospice, and primary care settings

Requirements

• Licensed as a NP in Texas
• Federal DEA License or eligibility
• Geriatric training or experience preferred
• Active CPR/BLS Certification
• Board Certification
• Valid driver’s license and good driving record
