Nurse Practitioner (NP) Value-based Primary Care

This Nurse Practitioner role at HarmonyCares provides value-based in-home primary care for a geriatric patient panel in northwest Chicago. The position follows a Monday-through-Friday schedule with no weekends or evenings. HarmonyCares operates home-based primary care practices across 14 states under an integrated team-based, physician-driven model.

Responsibilities

• Manage chronic conditions for an assigned panel of geriatric patients during home visits
• Provide primary care to high-acuity patients in their homes on a Monday-through-Friday schedule
• Focus on individualized patient care rather than visit volume targets
• Participate in an integrated, team-based, physician-driven care model

Requirements

• Active Nurse Practitioner license in Illinois
• Federal DEA license or eligibility
• Board certification as a Nurse Practitioner
• Active CPR/BLS certification
• Valid driver’s license with a good driving record
• Geriatric training or experience preferred
