Job Description

The PRN Nurse Practitioner conducts per-visit health risk assessments in residential settings or via telehealth for HarmonyCares, a national value-based provider of in-home primary care services for patients with complex healthcare needs. The role operates under a team-based, physician-driven model and is based in Indianapolis, Indiana. HarmonyCares delivers patient-centered care across multiple states with an emphasis on bringing personalized healthcare directly to patients' homes.

Responsibilities

  • Perform comprehensive in-home health risk assessments to identify active and chronic conditions along with physical, mental, and social needs
  • Obtain patient history, conduct examinations, and establish diagnoses within the Nurse Practitioner scope of practice
  • Document patient visits in accordance with NCQA standards
  • Measure and record vital signs when indicated
  • Initiate case management referrals and coordinate with primary care providers as needed
  • Communicate with patients, caregivers, agency nurses, other providers, and vendors to support accurate diagnosis and care
  • Adhere to OSHA Universal Precautions during all clinical activities
  • Protect patient confidentiality in all interactions
  • Attend required meetings, in-services, and committee participation as requested
  • Engage in professional development to maintain licenses and professional affiliations

Requirements

  • Active and unrestricted nurse practitioner license to practice in applicable coverage states
  • Board certification through ANCC, AANP, or NCCPA
  • Active BLS certification
  • Current enrollment in Medicare and Medicaid
  • Valid driver’s license with a good driving record
  • Proficiency with electronic health record systems
  • Geriatric training or experience
  • Ability to adapt to varying weather conditions and residential environments
  • Full range of body motion including patient handling and transfers, manual and finger dexterity, eye-hand coordination, normal visual acuity, normal hearing, standing, bending, walking, and stair climbing
  • Regular lifting and carrying of items weighing up to 50 pounds
  • Capacity to travel by automobile or van up to 150 miles daily in urban or rural settings
