The Community Liaison role at HarmonyCares supports a national home-based primary care practice operating through its family of companies that include medical group, home health, and hospice services. This position is based in Middleburg Heights, Ohio, and focuses on local market outreach to facilities, referral partners, and eligible patients who face access barriers to in-home care. The role involves both in-person and virtual engagement across an assigned territory.
Responsibilities
Conduct market analysis and competitive landscape reviews to differentiate services to external referral partners
Educate community partners on the mission to deliver high-quality personalized healthcare in the home for patients with difficult care access
Meet or exceed targets for in-person and virtual meetings with key decision makers
Explain eligibility criteria and enrollment processes for company programs
Facilitate benefit enrollment and related processes when requested
Track outreach performance in the CRM system against regional budget targets
Schedule and conduct face-to-face or phone discussions with patients, caregivers, or legal representatives to review program eligibility
Provide information on accountable care organization alignment or other CMS alternative payment models and collect Voluntary Alignment Forms from interested patients or representatives
Respond to inbound calls from patients, families, and facility staff regarding local business units
Triage patient concerns about care needs, scheduling, and billing to the appropriate internal department
Document outreach calls and meetings with external sources
Follow all company policies, procedures, and applicable laws related to marketing, referral interactions, beneficiary notifications, and patient outreach
Comply with HIPAA privacy and security guidelines
Maintain communication with sales leaders and local business unit leadership
Perform additional duties as assigned
Requirements
Bachelor’s degree or 4+ years of B2B sales experience
2+ years of healthcare industry sales or marketing experience
Valid driver’s license and good driving record
Proficiency with Microsoft Office applications including Word, Excel, and Outlook
Ability to travel within the assigned territory and attend required sales meetings
