Job Description
HarmonyCares operates as a national value-based provider of in-home primary care for patients with complex needs across 14 states, with headquarters in Troy, Michigan. The Nurse Practitioner PRN Telehealth position is located in Cleveland, Ohio, and serves patients anywhere in the state through telehealth or residential visits under a team-based, physician-driven model. The role focuses on annual risk assessments within the scope of practice delegated by the collaborative physician.
Responsibilities
Conduct comprehensive health risk assessments in residential settings or via telehealth to identify active and chronic conditions along with physical, mental, and social needs
Obtain patient history, perform examinations, and determine diagnoses
Provide written documentation of each patient visit according to NCQA standards
Measure and record patient vital signs when necessary
Place case management referrals and coordinate with primary care physicians as needed
Communicate with patients, caregivers, agency nurses, other providers, and vendors to support accurate diagnosis
Perform all clinical duties while following OSHA Universal Precautions
Maintain patient confidentiality at all times
Attend required meetings, in-services, and committee participation as requested
Engage in professional development activities and maintain required licenses and affiliations
Requirements
Active and unrestricted nurse practitioner license to practice in coverage states
Board certification through American Nurses Credentialing Center (ANCC), American Association of Nurse Practitioners (AANP), or National Commission on Certification of Physician Assistants (NCCPA)
Active BLS certification
Current enrollment in Medicare and Medicaid
Valid driver’s license with a good driving record
Proficiency with electronic health record systems
Ability to adapt to varying weather conditions and residential environments
Full range of body motion including handling and lifting patients, manual and finger dexterity, eye-hand coordination, normal visual acuity, normal hearing, standing, bending, walking, and stair climbing
Regular lifting and carrying of items weighing up to 50 pounds
Ability to ride in an automobile or van up to 150 miles daily in urban or rural settings and to drive when necessary
