Nurse Practitioner Primary Care Cleveland Ohio

This Nurse Practitioner Primary Care role is with HarmonyCares, a national value-based provider of in-home primary care services for patients with complex healthcare needs, based in Middleburg Heights, Ohio. The position delivers patient-centered care under an integrated, team-based, physician-driven model to high-acuity individuals in their homes. The schedule runs Monday through Friday with no holidays or hospital rounds and minimal call responsibilities.

Responsibilities

• Provide primary care to high-acuity patients in their homes Monday through Friday
• Focus on addressing individual patient needs without emphasis on volume

Requirements

• Licensed as a Nurse Practitioner in the State of Ohio
• Federal DEA License or eligibility
• Active CPR/BLS Certification
• Board Certification
• Valid driver’s license and good driving record
• Directly related experience in home health, geriatrics, hospice, or primary care preferred
• Geriatric training or experience preferred
