Nurse Practitioner (NP) Primary Care

HarmonyCares provides in-home primary care services for patients with complex healthcare needs as a national value-based provider operating in 14 states. The Nurse Practitioner Primary Care role in Federal Way, Washington, delivers care under an integrated, team-based, physician-driven model Monday through Friday with minimal call responsibilities and no hospital rounds or holidays. An assistant supports appointments in select markets, and the position targets high-acuity patients in their homes.

Responsibilities

• Provide primary care to high-acuity patients in their homes Monday through Friday
• Deliver patient-centered care with time allocated to address individual needs rather than visit volume
• Apply experience in home health, geriatrics, hospice, and primary care settings

Requirements

• Active NP license in the state where the HarmonyCares office is located
• Federal DEA license or eligibility
• Board certification
• Active CPR/BLS certification
• Valid driver’s license with a good driving record
• Geriatric training or experience preferred
