Nurse Practitioner (NP) Primary Care

HarmonyCares provides value-based in-home primary care to patients with complex healthcare needs in Hamilton Township, New Jersey. The Nurse Practitioner role operates under an integrated, team-based, physician-driven model and follows a Monday-through-Friday schedule with no hospital rounds. The position focuses on high-acuity patients in their homes with minimal call responsibilities.

Responsibilities

• Provide primary care services to high-acuity patients in their homes
• Practice under an integrated, team-based, physician-driven model

Requirements

• Licensed as a Nurse Practitioner in New Jersey
• Federal DEA License or eligibility
• Active CPR/BLS Certification
• Board Certification
• Valid driver’s license and good driving record
• Directly related experience in home health, geriatrics, hospice, or primary care
• Geriatric training or experience preferred
