Nurse Practitioner (NP) Primary Care

HarmonyCares operates a value-based in-home primary care practice serving patients with complex healthcare needs in Jersey City, New Jersey. The organization delivers care through an integrated, team-based, physician-driven model across 14 states. This full-time Nurse Practitioner role provides primary care Monday through Friday with no holidays or hospital rounds and minimal call responsibilities.

Responsibilities

• Deliver primary care visits to high-acuity patients in their homes
• Address individual patient needs without volume-based targets
• Document care and coordinate with the integrated care team

Requirements

• Active Nurse Practitioner license in New Jersey
• Federal DEA license or eligibility
• Board certification as a Nurse Practitioner
• Active CPR/BLS certification
• Valid driver’s license and acceptable driving record
• Geriatric training or experience preferred
