Job Description

HarmonyCares is a national value-based provider of in-home primary care services for patients with complex healthcare needs. The Medical Pod Leader position is based in Westbury, New York, and serves the Long Island or Queens area. This role functions as the on-site clinical leader for an assigned pod of advanced practice providers and physicians delivering care under a team-based, physician-driven model, with no hospital rounds or weekend/holiday requirements.

Responsibilities

  • Act as supervising and collaborating physician for assigned APPs and physicians, maintaining collaborative agreements within state limits
  • Complete at least one ride-along per quarter with each collaborative APP and provide complex patient visits as needed
  • Collaborate with clinical team members including Community Health Workers, Medical Social Workers, Nurse Navigators, Care Managers, and Patient Care Coordinators
  • Lead high-risk huddles and ensure documented plans of care for all discussed patients
  • Conduct 10 chart audits per month or per state regulations, whichever is higher
  • Perform panel reviews with APPs and sign DME, death certificates, and other orders as needed
  • Review quality metrics and pod-level performance to drive HEDIS outcomes
  • Review clinician capacity and productivity; partner with Site Medical Directors on countermeasures when providers are off track
  • Assist pod providers with EMR efficiency and participate in patient care for assigned panels, including high-risk or complex cases and cognitive assessments
  • Lead clinical education on accurate diagnosis, documentation, and chronic condition management
  • Handle discipline actions, performance reviews for direct reports, and assist with compliance and HR resolutions
  • Support service inquiry resolution and patient complaint handling for the pod as needed

Requirements

  • Bachelor’s degree
  • Doctor of Medicine or Osteopathy
  • Completion of an accredited residency program
  • Unrestricted medical license
  • Unrestricted DEA registration
  • Active CPR certification
  • Valid driver’s license with good driving record
  • Board certification required for all newly hired or promoted physicians on or after January 1, 2026
  • Three years of experience as a physician collaborating with advanced practice providers (preferred)
  • Experience in geriatrics or with complex patients (preferred)
