Job Description

HarmonyCares seeks a Physician Leader (MD/DO) for its value-based in-home primary care practice in Burr Ridge, Illinois. The role combines clinical oversight of advanced practice providers with direct chronic condition management for a small geriatric panel. The position follows a Monday-through-Friday schedule with no weekends or evenings and operates within a team-based, home-based care model serving patients with complex healthcare needs.

Responsibilities

  • Serve as on-site supervising and collaborating physician for assigned advanced practice providers and physicians within the pod, ensuring the total number of collaborative agreements complies with state regulations.
  • Conduct at least one ride-along with each collaborative APP per quarter and provide complex patient visits as needed.
  • Collaborate with community health workers, medical social workers, nurse navigators, care managers, and patient care coordinators on patient care plans.
  • Lead high-risk huddles and ensure documented care plans include input from all care management team members.
  • Perform 10 chart audits per month or the number required by state regulations, whichever is higher.
  • Review quality metrics and pod-level performance data to identify and implement improvements in HEDIS outcomes.
  • Monitor clinician capacity and productivity; partner with site medical directors to develop and apply corrective measures when performance falls short.
  • Conduct panel reviews with APPs, sign DME orders and death certificates as needed, and assist providers with EMR efficiency.
  • Participate in patient care for the assigned panel, including high-risk or complex visits, physician face-to-face requirements, and cognitive assessments.
  • Lead clinical education on accurate diagnosis, documentation, and management of chronic conditions.
  • Complete performance reviews, handle discipline actions, and support compliance and HR resolutions for direct reports.
  • Assist with service inquiries and resolution of patient complaints for the pod as needed.

Requirements

  • Doctor of Medicine or Osteopathy degree from an accredited institution.
  • Completion of an accredited residency program.
  • Active, unrestricted medical license.
  • Unrestricted DEA registration.
  • Active CPR certification.
  • Valid driver’s license with a good driving record.
  • Board certification required for all physicians hired or promoted on or after January 1, 2026, and must be maintained in accordance with organizational and regulatory standards.
  • Minimum three years of experience as a physician collaborating with advanced practice providers (preferred).
  • Experience in geriatrics or management of complex patients (preferred).
