HarmonyCares seeks a full-time Nurse Practitioner to deliver in-home primary care to high-acuity patients with complex healthcare needs in Whitestone, New York. The role operates within a value-based, home-based primary care practice that follows an integrated, team-based, physician-driven model across multiple states. The position follows a Monday-through-Friday schedule with no hospital rounds and minimal call responsibilities.

Responsibilities

• Provide primary care to high-acuity patients in their homes Monday through Friday
• Deliver patient-centered care under an integrated, team-based, physician-driven model
• Allocate time to address individual patient needs rather than focusing on visit volume

Requirements

• Licensed as a Nurse Practitioner in New York
• Federal DEA License or eligibility
• Board Certification
• Active CPR/BLS Certification
• Valid driver’s license and good driving record
• Geriatric training or experience preferred
