Job Description

The Licensed Clinical Social Worker position at HarmonyCares supports care coordination for patients with complex healthcare needs in a hybrid model (90% remote or telephonic and 10% in-field visits) located in Jacksonville, Florida. HarmonyCares operates as a national value-based provider of in-home primary care services across 14 states. The role focuses on patients who face barriers to accessing traditional care settings.

Responsibilities

  • Manage a caseload of high-risk patients presenting with significant social complexities
  • Perform psychosocial assessments through both in-person visits and telephone contacts with patients or caregivers
  • Identify psychological and social determinant barriers and create targeted interventions to address gaps in care
  • Deliver short-term counseling services when indicated
  • Coordinate services so that patients and families understand their treatment plans
  • Participate as an active member of the collaborative care team, including High-Risk Huddles, to support quality and patient experience outcomes
  • Conduct outreach to community-based services that enable patients to remain at home
  • Develop and implement shared goals of care with patients and families
  • Build relationships with community agencies to connect patients with resources that support independence

Requirements

  • Master’s degree in social work
  • Current LCSW or LISW license (state-dependent)
  • Valid driver’s license with an acceptable driving record
  • Above-average computer skills
  • Experience in care management within a community-based setting or equivalent setting (preferred)
  • Experience working with chronic, complex, and/or behavioral health populations (preferred)
