Job Description

HarmonyCares is a national value-based provider of in-home primary care for patients with complex healthcare needs. The Phlebotomist position is based in Whitestone, New York, and supports the HarmonyCares Medical Group physician model by performing specimen collection and related tasks during daytime visits in residential settings. The role involves travel of up to 150 miles per day with no holidays or weekends required.

Responsibilities

  • Identify optimal methods for retrieving blood samples and other specimens for laboratory testing and perform screening procedures
  • Verify test requisitions against clinician orders and escalate discrepancies to leadership
  • Confirm patient identification and explain sampling procedures
  • Perform venipunctures and finger sticks to obtain blood specimens
  • Maintain specimen integrity through aseptic technique, department procedures, and isolation protocols
  • Track collected specimens by initialing, dating, noting collection times, and maintaining daily tallies
  • Follow standards and procedures to maintain a safe work environment and comply with legal regulations
  • Adhere to professional standards, institutional policies, and federal, state, and local requirements
  • Take vital signs as needed
  • Label, store, and package specimens for lab delivery
  • Log patient information for accurate processing and testing in compliance with CLIA standards
  • Observe OSHA and HIPAA requirements during all clinical duties
  • Prepare and maintain supplies and equipment for treatment, including supply inventory, medical bag, and vehicle
  • Facilitate required consent form signatures
  • Attend required meetings and in-services

Requirements

  • High School Diploma
  • Phlebotomist Certification
  • CPR Certification
  • Valid driver’s license and good driving record
  • Ability to drive up to 150 miles daily in urban and/or rural settings
  • Full range of body motion including handling and lifting patients
  • Ability to lift and carry items weighing up to 50 lbs
  • Ability to read and understand road maps
  • Ability to sit or stand for long periods of time
  • Proficiency in venipuncture, phlebotomy, and finger sticks
  • Familiarity with electronic medical records (EMR)
  • Experience with iPad and GPS technology
  • Six months of experience in patient care, preferably geriatric care
