Job Description

HarmonyCares provides in-home primary care services for patients with complex healthcare needs across 14 states. The Physician Leader - Primary Care role is based in Richmond, Virginia, and serves as the clinical leader for a pod team that includes advanced practice providers and other clinical staff. The position operates under a team-based, physician-driven model focused on home visits with no hospital rounds or weekend or holiday shifts required.

Responsibilities

  • Serve as on-site supervising and collaborating physician for assigned advanced practice providers and physicians within the pod while staying within state regulatory limits on collaborative agreements
  • Conduct at least one ride-along observation with each collaborative advanced practice provider per quarter and provide complex patient visits as needed
  • Collaborate with community health workers, medical social workers, nurse navigators, care managers, and patient care coordinators on patient care plans
  • Lead high-risk patient huddles and ensure documented care plans include input from all relevant team members
  • Perform at least 10 chart audits per month or the number required by state regulations, whichever is higher
  • Review quality metrics and pod-level performance data to identify and implement improvements targeting HEDIS measures
  • Monitor clinician capacity and productivity and partner with site medical directors on corrective actions when targets are missed
  • Assist pod providers with EMR efficiency and sign DME orders, death certificates, and other documents as permitted by state regulation
  • Conduct routine panel reviews with advanced practice providers and participate in difficult patient or family conversations when requested
  • Deliver clinical education on accurate diagnosis, documentation, and chronic condition management
  • Handle discipline actions and performance reviews for direct reports and support compliance and HR resolutions as needed
  • Provide direct patient care for assigned panel members and additional high-risk or complex cases, including physician face-to-face requirements and cognitive assessments

Requirements

  • Bachelor’s degree and completion of an accredited residency program
  • Doctor of Medicine or Doctor of Osteopathy degree
  • Unrestricted medical license and unrestricted DEA registration
  • Active CPR certification and valid driver’s license with acceptable driving record
  • Board certification required for all physicians hired or promoted on or after January 1, 2026
  • At least three years of experience as a physician collaborating with advanced practice providers preferred
  • Experience with geriatric or complex patient populations preferred
