PRN Nurse Practitioner, Homevisits/Telehealth

HarmonyCares is hiring a PRN Nurse Practitioner to perform home visits and telehealth services in Lexington, Kentucky. The position supports a national value-based provider of in-home primary care for patients with complex healthcare needs through an integrated team-based physician-driven model. The role focuses on annual risk assessments in residential settings or via telehealth under the delegation of a Collaborative Physician, with no holidays or hospital rounds.

Responsibilities

• Conduct comprehensive in-home health risk assessments to identify all active and chronic disease conditions as well as determine all physical, mental, and social needs present at the time of the visit
• Take history, examine patients, and determine diagnoses
• Provide written documentation of patient visits per NCQA standards
• Take patient vital signs as necessary
• Place case management referrals and communicate with PCP as necessary
• Communicate with patients, caregivers, agency nurses, other providers, and vendors as necessary to assure proper diagnosis
• Perform all clinical duties while observing OSHA Universal Precautions
• Maintain patient confidentiality
• Attend required meetings and in-services and participate in committees as requested
• Participate in professional development activities and maintain professional licenses and affiliations

Requirements

• Active/unrestricted nurse practitioner license to practice in coverage states
• Board certification in one of the following: American Nurses Credentialing Center (ANCC), American Association of Nurse Practitioners (AANP), or National Commission on Certification of Physician Assistants (NCCPA)
• Active BLS Certification
• Current enrollment in Medicare/Medicaid
• Valid driver’s license and good driving record
• EHR skills
• Geriatric training/experience
• Adaptability to differing weather conditions and patients’ home/residential environments
• Full range of body motion including handling/lifting patients, manual and finger dexterity, eye-hand coordination, normal visual acuity, normal hearing, standing, bending, walking, and stair climbing
• Regular lifting/carrying items weighing up to 50 pounds
• Ability to ride in automobile or van up to 150 miles daily in urban and/or rural settings and ability to drive if necessary
