Nurse Practitioner (NP) full time - Sandy Springs - Primary Care

HarmonyCares is a national value-based provider of in-home primary care services for patients with complex healthcare needs. The organization runs home-based practices across 14 states using a team-based, physician-driven model. This full-time Nurse Practitioner role is based in Sandy Springs, Atlanta, Georgia, and centers on Monday-through-Friday primary care visits in patient homes with no hospital rounds or holidays.

Responsibilities

• Deliver primary care to high-acuity patients in their homes on a Monday-through-Friday schedule
• Focus clinical time on addressing individual patient needs rather than visit volume
• Operate under an integrated, team-based, physician-driven care model

Requirements

• Active Nurse Practitioner license in the state of Georgia
• Federal DEA license or eligibility
• Board certification as a Nurse Practitioner
• Active CPR/BLS certification
• Geriatric training or experience preferred
• Valid driver’s license with a good driving record
