Job Description

HarmonyCares is hiring a Physician Leader (MD/DO) Primary Care to build and lead a clinical pod team in the McAllen, Texas area, serving Hidalgo, Texas. The position oversees value-based in-home primary care for high-risk Medicare patients with chronic conditions, Monday through Friday, as part of a national organization with more than 250 providers across 14 states. The role includes dyad partnership with the Market Manager and functions as the collaborating physician for assigned advanced practice providers.

Responsibilities

  • Act as on-site supervising and collaborating physician for assigned APPs and physicians within the pod, ensuring collaborative practice agreements do not exceed state regulations
  • Participate in at least one ride-along with each collaborative APP per quarter and provide complex patient visits as needed
  • Collaborate with clinical team members including Community Health Workers, Medical Social Workers, Nurse Navigators, Care Managers, and Patient Care Coordinators
  • Develop APP clinical skills in evidenced-based care and lead high-risk huddles with documented care plans
  • Conduct at least 10 chart audits per month or per state regulations, whichever is higher
  • Review quality metrics and pod-level performance to drive HEDIS outcomes and identify improvement areas
  • Review clinician capacity and productivity; partner with Site Medical Directors on countermeasures when providers are off track
  • Work with the Regional Medical Director to identify care improvement opportunities
  • Assist with patient complaint resolution, service inquiries, and difficult patient or family conversations
  • Support EMR efficiency for pod providers and sign DME, death certificates, and other orders as needed
  • Participate in patient care for the assigned panel, including high-risk or complex visits, physician face-to-face requirements, and cognitive assessments
  • Lead clinical education on accurate diagnosis, documentation, and chronic condition management
  • Handle discipline actions, performance reviews for direct reports, and assist with compliance and HR resolutions
  • Perform additional duties assigned by the Regional Medical Director related to pod performance and outcomes

Requirements

  • Bachelor’s Degree
  • Doctor of Medicine or Osteopathy
  • Completion of an accredited Residency Program
  • Unrestricted Medical License
  • Board certification required for all newly hired or promoted physicians on or after January 1, 2026
  • Unrestricted DEA
  • Active CPR Certification
  • Valid driver’s license with good driving record
  • 3 years of experience as a Physician collaborating with Advance Practice Providers (preferred)
  • Experience in Geriatrics or complex patients (preferred)
