Nurse Practitioner (NP) Primary Care

This Nurse Practitioner Primary Care role is with HarmonyCares, a national value-based provider of in-home primary care services for patients with complex healthcare needs. The position is located in Ann Arbor, Michigan, and operates under an integrated, team-based, physician-driven model with more than 200 primary care providers across 14 states. The schedule runs Monday through Friday from 8:00 AM to 4:30 PM with no weekends or evenings, managing 6-8 patients per day focused on chronic conditions and supported by Athena AI Scribe for same-day charting. HarmonyCares centers its approach on delivering personalized, quality-based healthcare directly in patients' homes.

Responsibilities

• Provide primary care to high-acuity patients in their homes Monday through Friday
• Manage chronic conditions for 6-8 patients per day
• Use Athena AI Scribe to complete charting within the workday
• Focus on patient care needs rather than visit volume in a home health, geriatrics, hospice, or primary care setting

Requirements

• Licensed as a Nurse Practitioner in the state where the HarmonyCares Medical Group office is located
• Federal DEA License or eligibility
• Board Certification
• Active CPR/BLS Certification
• Valid driver’s license and good driving record
• Geriatric training or experience preferred
