Job Description

The PRN Nurse Practitioner - Per Visit role at HarmonyCares supports in-home primary care services for patients with complex healthcare needs in Indianapolis, Indiana. HarmonyCares operates home-based practices across multiple states under an integrated, team-based, physician-driven model. This per-visit position focuses on annual risk assessments conducted in residential settings or via telehealth.

Responsibilities

  • Conduct comprehensive in-home health risk assessments to identify active and chronic disease conditions along with physical, mental, and social needs
  • Obtain patient history, perform examinations, and determine diagnoses within the nurse practitioner scope of practice
  • Document patient visits in accordance with NCQA standards
  • Measure and record patient vital signs as needed during visits
  • Place case management referrals and coordinate with primary care physicians as required
  • Communicate with patients, caregivers, agency nurses, providers, and vendors to support accurate diagnosis and care
  • Adhere to OSHA Universal Precautions during all clinical activities
  • Maintain patient confidentiality at all times
  • Attend required meetings, in-services, and committee participation as requested
  • Engage in professional development activities and sustain required licenses and affiliations

Requirements

  • Active and unrestricted nurse practitioner license in applicable coverage states
  • Board certification through ANCC, AANP, or NCCPA
  • Active BLS certification
  • Current enrollment in Medicare and Medicaid
  • Valid driver’s license with acceptable driving record
  • Proficiency with electronic health record systems
  • Geriatric training or experience
  • Ability to adapt to varying weather conditions and residential environments
  • Full range of body motion including patient handling and lifting, manual dexterity, eye-hand coordination, normal visual acuity and hearing, standing, bending, walking, and stair climbing
  • Capability to lift and carry items weighing up to 50 pounds regularly
  • Ability to travel up to 150 miles daily by automobile in urban or rural settings
