Physician Leader (MD /DO) Value-based Primary Care

Job Description

HarmonyCares is a national value-based provider of in-home primary care services for patients with complex healthcare needs, headquartered in Troy, Michigan, and operating practices across 14 states. The Physician Leader (MD/DO) role combines direct management of a small chronic-condition panel with leadership of high-risk huddles and clinical oversight of advanced practice providers. The position operates on a Monday-through-Friday schedule with no weekends, holidays, or hospital rounds and maintains a dyad partnership with the Market Manager.

Responsibilities

  • Act as on-site supervising physician for assigned APPs and physicians within the pod, ensuring the total number of collaborative practice agreements does not exceed state regulations
  • Complete at least one ride-along with each collaborative APP per quarter and provide complex patient visits as needed for supervised APPs
  • Collaborate with community health workers, medical social workers, nurse navigators, care managers, and patient care coordinators
  • Lead and participate in high-risk huddles, ensuring documented care plans include all care-management team members
  • Conduct 10 chart audits per month or the number required by state regulations, whichever is higher
  • Perform routine panel reviews with APPs and sign DME, death certificates, and other orders as needed on behalf of APPs per state regulations
  • Review quality metrics and pod-level performance to identify improvements in HEDIS outcomes
  • Review clinician capacity and productivity, partnering with site medical directors to implement countermeasures when providers are off track
  • Work with the regional medical director to identify opportunities for care improvement
  • Assist with service inquiries, resolution of patient complaints, and requests for the pod
  • Support pod providers in achieving higher EMR efficiency
  • Participate in patient care for the assigned panel and other high-risk or complex patients as needed, including physician face-to-face requirements and cognitive assessments
  • Lead clinical education on accurate diagnosis, documentation, and management of chronic conditions
  • Assist with compliance and HR resolutions, including discipline actions and performance reviews for direct reports
  • Complete additional duties assigned by the regional medical director related to pod performance and outcomes

Requirements

  • Bachelor’s degree
  • Doctor of Medicine or Osteopathy
  • Completion of an accredited residency program
  • Unrestricted medical license
  • Unrestricted DEA registration
  • Active CPR certification
  • Valid driver’s license with a good driving record
  • Board certification required for all newly hired or promoted physicians on or after January 1, 2026, and must be maintained in accordance with organizational and regulatory standards
  • 3 years of experience as a physician collaborating with advanced practice providers (preferred)
  • Experience in geriatrics or with complex patients (preferred)
