Medical Assistant (Clinical Partner)

The Medical Assistant (Clinical Partner) role is with HarmonyCares, a national provider of in-home primary care services for patients with complex healthcare needs, based in Uniontown, Ohio. The position supports primary care physicians and nurse practitioners in residential settings under a team-based model. The schedule consists of daytime hours with no weekends or holidays.

Responsibilities

• Assist primary care physicians, nurse practitioners, and care management teams with in-home primary care delivery and patient care management
• Coordinate daily travel plans and transport clinicians to patient appointments
• Prepare patients for examinations, treatments, and minor procedures in residential settings
• Obtain patient consent forms and document care accurately and promptly in the electronic medical record
• Perform phlebotomy via venipuncture, EKGs, and other procedures as directed by clinicians
• Label, store, and package specimens for laboratory transport
• Update patient care plans, social determinants of health assessments, and other documentation in the chart or EMR
• Maintain compliance with CLIA, OSHA, and HIPAA guidelines
• Manage supply inventory and prepare equipment for in-home visits, including the medical bag and vehicle
• Perform periodic vehicle maintenance and ensure cleanliness of the company vehicle
• Attend required meetings and training sessions

Requirements

• High school diploma or GED equivalent
• Graduation from an accredited Medical Assistant, phlebotomy, EMT, or related medical training program, or one year of experience as a Medical Assistant or phlebotomist
• Current CPR certification meeting company guidelines
• Knowledge of medical terminology
• Basic computer literacy including EMR, iPad, and GPS use
• Valid driver’s license and good driving record
• Full range of body motion including patient handling and transfers
• Ability to drive up to 150 miles daily in urban or rural areas
• Ability to lift and carry items up to 50 pounds
• Ability to read and understand road maps
• Capacity to sit or stand for extended periods
