Nurse Practitioner (NP) Value-based Primary Care role at HarmonyCares in Spring, Texas. The position focuses on delivering primary care to high-acuity patients in their homes on a Monday-through-Friday schedule with minimal call responsibilities.

Responsibilities

• Deliver in-home primary care services to high-acuity patients Monday through Friday
• Focus on patient care needs rather than visit volume targets
• Apply experience in home health, geriatrics, hospice, and primary care settings

Requirements

• Active NP license in the state where the HarmonyCares office is located
• Federal DEA license or eligibility
• Board certification as a Nurse Practitioner
• Active CPR/BLS certification
• Valid driver’s license with a good driving record
• Geriatric training or experience preferred
