Advance Practice Provider (NP /PA) Value-based Primary Care

HarmonyCares seeks an Advance Practice Provider (NP or PA) for a value-based primary care role in Bastrop, Texas. The position involves delivering in-home primary care to geriatric patients with complex healthcare needs under an integrated, team-based model. The schedule is Monday through Friday with no weekends, evenings, holidays, or hospital rounds. Minimal call responsibilities apply.

Responsibilities

• Manage chronic conditions for an assigned panel of geriatric patients
• Provide primary care to high-acuity patients during in-home visits Monday through Friday
• Focus on addressing individual patient needs without emphasis on visit volume

Requirements

• Licensed as a NP or PA in Texas
• Federal DEA License or eligibility
• Active CPR/BLS Certification
• Board Certification
• Valid driver’s license and good driving record
• Geriatric training or experience preferred
