Job Description

Physician Leader (MD/DO) role in value-based primary care at HarmonyCares in Portage, Michigan. The position provides clinical oversight for advanced practice providers managing chronic conditions for high-risk geriatric patients through in-home visits on a Monday-to-Friday schedule with no weekends or evenings. HarmonyCares delivers home-based primary care across 14 states under an integrated, team-based model for patients with complex needs who have limited access to office-based services.

Responsibilities

  • Act as on-site supervising and collaborating physician for assigned advanced practice providers and physicians, maintaining collaborative practice agreements within state limits
  • Complete at least one ride-along with each collaborative APP per quarter and provide complex patient visits as needed
  • Collaborate with community health workers, medical social workers, nurse navigators, care managers, and patient care coordinators on patient plans
  • Lead high-risk huddles and ensure documented care plans that include all care management team members
  • Conduct 10 chart audits per month or the number required by state regulations, whichever is higher
  • Review quality metrics and pod-level performance to identify improvements in HEDIS measures
  • Review clinician capacity and productivity; partner with site medical directors to implement countermeasures when performance lags
  • Sign DME orders, death certificates, and other documents as required by state regulation on behalf of APPs
  • Participate in patient care for the assigned panel and other high-risk or complex cases, including cognitive assessments and physician face-to-face requirements
  • Lead clinical education on accurate diagnosis, documentation, and management of chronic conditions
  • Complete performance reviews and disciplinary actions for direct reports
  • Assist with service inquiries, patient complaint resolution, and compliance matters as needed

Requirements

  • Bachelor’s degree
  • Doctor of Medicine or Osteopathy
  • Completion of an accredited residency program
  • Unrestricted medical license
  • Board certification required for all newly hired or promoted physicians on or after January 1, 2026; must obtain and maintain certification per organizational and regulatory standards
  • Unrestricted DEA registration
  • Active CPR certification
  • Valid driver’s license with good driving record
  • Three years of experience as a physician collaborating with advanced practice providers (preferred)
  • Experience in geriatrics or with complex patients (preferred)
