Nurse Practitioner (NP) Primary Care

The Nurse Practitioner Primary Care role is with HarmonyCares in Flint, Michigan. HarmonyCares delivers in-home primary care services for patients with complex needs under a value-based model and operates practices in 14 states. This position focuses on geriatric chronic condition management Monday through Friday with no weekends or evenings, 30-45 minute patient visits, and charting completed during the workday using Athena and AI Scribe.

Responsibilities

• Provide in-home primary care to high-acuity patients with time allocated to address individual needs rather than visit volume
• Manage geriatric chronic conditions within a value-based care framework
• Complete patient documentation during scheduled work hours using Athena and AI Scribe tools

Requirements

• Active Nurse Practitioner license in Michigan
• Federal DEA license or eligibility
• Board certification as a Nurse Practitioner
• Active CPR/BLS certification
• Valid driver’s license with acceptable driving record
• Preferred: prior experience or training in geriatrics, home health, hospice, or primary care
