Medical Assistant ( Clinical Partner)

The Clinical Partner (Medical Assistant) role at HarmonyCares supports delivery of in-home primary care services to patients with complex healthcare needs in Newark, New Jersey. HarmonyCares operates home-based primary care practices under an integrated, team-based model and maintains daytime schedules with no holidays or weekends. The position requires daily travel to residential appointments within a manageable caseload.

Responsibilities

• Coordinate daily travel and transport clinicians to patient appointments
• Prepare patients for examination, treatment, and minor procedures
• Obtain patient consent forms and document care in the electronic medical record in a timely manner
• Perform phlebotomy via venipuncture, EKGs, and other procedures as directed
• Label, store, and package specimens for laboratory transport
• Update patient care plans, Social Determinants of Health assessments, and related documentation
• Maintain compliance with CLIA, OSHA, and HIPAA guidelines
• Manage supply inventory and prepare medical equipment and bags for daily use
• Perform routine maintenance and cleaning of the company vehicle
• Attend required meetings and training sessions

Requirements

• High school diploma or GED equivalent
• Completion of an accredited Medical Assistant, Phlebotomy, or EMT program, or one year of experience as a Medical Assistant or Phlebotomist
• Current CPR certification meeting company standards
• Knowledge of medical terminology
• Basic computer literacy including EMR, iPad, and GPS use
• Valid driver’s license and acceptable driving record
• Ability to perform full range of body motion including patient handling and transfers
• Ability to lift and carry items up to 50 pounds
• Ability to drive up to 150 miles daily in urban or rural settings
• Ability to read and interpret road maps
• Capacity to sit or stand for extended periods
