Advance Practice Provider (NP/PA) Primary Care

HarmonyCares is hiring an Advance Practice Provider (NP/PA) for primary care in Dripping Springs, Texas. The position supports in-home care for high-risk Medicare patients with complex conditions as part of a value-based model serving over 85,000 patients across multiple states. Providers manage their own geriatric panels with 30-45 minute appointments and may choose either a 32-hour (4-day) or 40-hour (5-day) schedule, Monday through Friday with no weekends or evenings.

Responsibilities

• Deliver primary care and chronic condition management to high-acuity geriatric patients during in-home visits
• Conduct 30-45 minute appointments focused on individual patient needs under a value-based care approach
• Work within an integrated team-based model that includes physician oversight and support staff where available

Requirements

• Active NP or PA license in Texas
• Federal DEA license or eligibility
• Active CPR/BLS certification
• Board certification
• Valid driver’s license and acceptable driving record
• Geriatric training or experience preferred
