Medical Assistant (Clinical Partner)

The Medical Assistant (Clinical Partner) role at HarmonyCares supports delivery of in-home primary care services for patients with complex healthcare needs. The position is located in Oakbrook Terrace, Illinois, and operates on daytime hours with no weekends or holidays in a home-based setting with manageable visit volumes.

Responsibilities

• Assist primary care physicians, nurse practitioners, and clinical teams with patient care management in residential settings.
• Coordinate daily travel itineraries and transport clinicians to scheduled appointments.
• Prepare patients for examination and treatment, including minor procedures when needed.
• Obtain patient consent forms and record care documentation accurately and on time.
• Coordinate patient care according to clinician direction, company standards, and policies.
• Perform phlebotomy via venipuncture, EKG, and other procedures as requested by clinicians.
• Label, store, and package specimens properly for laboratory delivery.
• Identify and close quality gaps in patient care.
• Document and update patient care plans, Social Determinants of Health assessments, and other evaluations in the electronic medical record.
• Maintain compliance with CLIA, OSHA, and HIPAA guidelines.
• Prepare and maintain supplies and equipment for treatment, including supply inventory, medical bag, and vehicle where applicable.
• Ensure company vehicle cleanliness and adherence to maintenance schedules where applicable.
• Attend required meetings and in-services.

Requirements

• High school diploma or GED equivalent.
• Graduate of an accredited Medical Assistant, Phlebotomy, EMT, or related medical training program, or one year of experience as a Medical Assistant or Phlebotomist.
• Current CPR certification meeting company guidelines.
• Knowledge of medical terminology.
• Basic computer literacy including EMR, iPad, and GPS use.
• Valid driver’s license with good driving record.
• Full range of body motion including handling and lifting patients.
• Ability to drive up to 150 miles daily in urban or rural settings.
• Ability to lift and carry items up to 50 lb.
• Ability to read and understand road maps.
• Ability to sit or stand for long periods.
• Certified Medical Assistant credential preferred.
• Six months of patient care experience, preferably in geriatric settings, preferred.
• Strong phlebotomy skills preferred.
• Ability to obtain MA certification within 180 days of hire preferred.
