Nurse Practitioner (NP) Primary Care

HarmonyCares offers a Nurse Practitioner primary care position delivering in-home services to patients with complex healthcare needs in Lancaster, Pennsylvania. The role is part of a value-based home-based primary care practice that operates in 14 states under an integrated, team-based, physician-driven model. Patient visits occur Monday through Friday.

Responsibilities

• Provide primary care to high-acuity patients in their homes
• Allocate sufficient time to address individual patient needs rather than focusing on visit volume

Requirements

• Licensed as a Nurse Practitioner in Pennsylvania
• Federal DEA license or eligibility
• Board certification
• Active CPR/BLS certification
• Geriatric training or experience preferred
• Valid driver’s license and good driving record
