Nurse Practitioner (NP) Primary Care

HarmonyCares seeks a Nurse Practitioner for a primary care role serving patients with complex healthcare needs in the Denton, Texas area. The position operates within a national value-based, in-home primary care model that uses an integrated, team-based, physician-driven approach. The schedule runs Monday through Friday with no weekends or evenings, and the role covers a patient panel across Denton, Aubrey, Lewisville, and Carrollton with 30-45 minute visits at a volume of approximately 6-9 patients per day.

Responsibilities

• Deliver in-home primary care to high-acuity patients with complex needs under a value-based care model
• Conduct patient visits of 30-45 minutes while managing a daily volume of 6-9 patients
• Focus on individualized patient care rather than visit volume targets
• Provide services Monday through Friday with no hospital rounds or holiday coverage

Requirements

• Licensed as a Nurse Practitioner in Texas
• Federal DEA License or eligibility
• Board Certification
• Active CPR/BLS Certification
• Valid driver’s license with a good driving record
• Geriatric training or experience preferred
