Nurse Practitioner (NP) Primary Care - Part-time

HarmonyCares is hiring a part-time Nurse Practitioner for Primary Care focused on geriatric chronic condition management. The role delivers in-home primary care under a value-based model to high-acuity patients in the Kent, Federal Way, Auburn, and Renton areas of Washington on a set schedule of Monday, Tuesday, and Friday. HarmonyCares operates home-based primary care practices in 14 states using an integrated, team-based, physician-driven approach.

Responsibilities

• Deliver in-home primary care to high-acuity patients with complex healthcare needs using a value-based care model
• Focus on geriatric chronic condition management during scheduled patient visits
• Provide care in the Kent, Federal Way, Auburn, and Renton service areas
• Follow a part-time schedule of Monday, Tuesday, and Friday with no hospital rounds or holidays

Requirements

• Licensed as a Nurse Practitioner in Washington
• Federal DEA License or eligibility
• Active CPR/BLS Certification
• Board Certification
• Valid driver’s license and good driving record
• Geriatric training or experience preferred
