Nurse Practitioner (NP) In-Home Primary Care

HarmonyCares is hiring a Nurse Practitioner for in-home primary care in Paterson, New Jersey. The position provides patient-centered primary care to individuals with complex healthcare needs through a home-based, team-based, physician-driven model within an Accountable Care Organization. Care is delivered Monday through Friday with no hospital rounds or holidays.

Responsibilities

• Provide primary care to high-acuity patients in their homes Monday through Friday
• Allocate time to address individual patient needs without emphasis on visit volume

Requirements

• Licensed as a Nurse Practitioner in New Jersey
• Federal DEA License or eligibility
• Board Certification
• Active CPR/BLS Certification
• Valid driver’s license and good driving record
• Spanish speaking preferred
• Geriatric training or experience preferred
