Nurse Practitioner Primary Care- East Suffolk/ Long Island NY

HarmonyCares is a national value-based provider of in-home primary care services for patients with complex healthcare needs. This Nurse Practitioner Primary Care position is based in Westbury, New York, serving the East Suffolk and Long Island area. Clinicians deliver patient-centered care under an integrated, team-based, physician-driven model, with primary care visits scheduled Monday through Friday and no hospital rounds or holidays required.

Responsibilities

• Provide in-home primary care to high-acuity patients Monday through Friday
• Focus on patient-centered visits that address individual needs rather than visit volume
• Participate in an integrated team-based model with physician oversight

Requirements

• Active Nurse Practitioner license in New York
• Federal DEA license or eligibility
• Board certification as a Nurse Practitioner
• Active CPR/BLS certification
• Valid driver’s license with good driving record
• Geriatric training or experience preferred
