Physician Leader in Springfield, VA

The Medical Pod Leader role at HarmonyCares is a home-based primary care position in Springfield, Virginia. HarmonyCares delivers value-based in-home primary care services to patients with complex healthcare needs across 14 states under a team-based, physician-driven model. The position serves as clinical leader for a pod team, functions as collaborating physician for advanced practice providers, and maintains a dyad partnership with the Market Manager, with no hospital rounds or weekend or holiday shifts required.

Responsibilities

• Serve as on-site supervising physician for assigned advanced practice providers and physicians within the pod while ensuring the total number of collaborative practice agreements complies with state regulations
• Conduct at least one ride-along with each collaborative advanced practice provider per quarter and provide complex patient visits as needed for supervised providers
• Collaborate with pod clinical team members including community health workers, medical social workers, nurse navigators, care managers, and patient care coordinators
• Support clinical development of advanced practice providers through evidenced-based care practices and professionalism guidance
• Lead high-risk huddles, confirm documented plans of care for all discussed patients, and include all care management team members
• Perform 10 chart audits per month or the number required by state regulations, whichever is higher
• Act as a resource for clinical team members on patient care questions and participate in difficult patient or family conversations when requested
• Conduct routine panel reviews with advanced practice providers
• Sign durable medical equipment orders, death certificates, and other documents on behalf of advanced practice providers as permitted by state regulation
• Review quality metrics and pod-level performance data, identify improvement areas, and drive superior HEDIS outcomes
• Review clinician capacity and productivity performance, develop countermeasures with site medical directors when providers fall off track, and support implementation
• Partner with the regional medical director to identify care improvement opportunities
• Assist with service inquiries, resolve patient complaints, and handle pod-related requests as needed
• Help pod providers achieve greater electronic medical record efficiency
• Deliver direct patient care to the assigned panel and other patients as needed, including high-risk or complex cases, physician face-to-face requirements, and cognitive assessments
• Lead clinical education on accurate diagnosis, documentation, and management of chronic conditions
• Support compliance and human resources resolutions and carry out discipline actions and performance reviews for direct reports
• Complete additional duties assigned by the regional medical director related to pod performance and outcomes

Requirements

• Bachelor's degree
• Doctor of Medicine or Osteopathy degree
• Valid driver's license with a good driving record
• Active CPR certification
• Unrestricted DEA registration
• Completion of an accredited residency program
• Unrestricted medical license
• Board certification required for all newly hired or promoted physicians on or after January 1, 2026, with ongoing maintenance in accordance with organizational and regulatory standards
• Three years of experience as a physician collaborating with advanced practice providers (preferred)
• Experience in geriatrics or with complex patients (preferred)
