Advance Practice Provider (NP /PA) Primary Care

HarmonyCares delivers value-based in-home primary care for patients with complex healthcare needs through a team-based, physician-driven model. This Advance Practice Provider (NP/PA) Primary Care position is located in Mesquite, Texas, and focuses on geriatric patients with chronic conditions in the East Dallas area. The role follows a Monday-Friday schedule with no weekends or evenings.

Responsibilities

• Provide primary care to high-acuity patients in their homes Monday through Friday

Requirements

• Licensed as a NP or PA in the State of Texas
• Federal DEA License or eligibility
• Active CPR/BLS Certification
• Board Certification
• Valid driver’s license and good driving record
• Geriatric training or experience preferred
