Nurse Practitioner (NP) Primary Care

Nurse Practitioner (NP) Primary Care position with HarmonyCares, a national value-based provider of in-home primary care services for patients with complex healthcare needs. The role is based in Troy, Michigan, and operates under an integrated team-based physician-driven model across home-based practices in 14 states. The schedule covers Monday through Friday in-home visits to high-acuity patients with no holidays or hospital rounds and minimal call responsibilities.

Responsibilities

• Deliver in-home primary care with emphasis on addressing individual patient needs
• Focus clinical time on direct patient care rather than service volume

Requirements

• Licensed as a NP in the State where HarmonyCares Medical Group office is located
• Federal DEA License or eligibility
• Geriatric training or experience preferred
• Active CPR/BLS Certification
• Board Certification
• Valid driver’s license and good driving record
