HarmonyCares is a national value-based provider of in-home primary care services for patients with complex healthcare needs. The Physician Leader (Medical Pod Leader) position is based in Springfield, Virginia, and serves as the on-site clinical leader for an assigned pod team that includes advanced practice providers and other clinical staff. The role operates under a team-based, physician-driven model with a dyad partnership alongside the Market Manager, and includes no holidays, weekends, or hospital rounds.
Responsibilities
Act as supervising and collaborating physician for assigned APPs and physicians within the pod, ensuring collaborative practice agreements do not exceed state regulations
Complete at least one ride-along with each collaborative APP per quarter
Provide complex patient visits as needed for supervised APPs
Collaborate with community health workers, medical social workers, nurse navigators, care managers, and patient care coordinators
Lead and participate in high-risk huddles with documented plans of care involving all team members
Conduct 10 chart audits per month or per state regulations, whichever is higher
Review quality metrics and pod-level performance to identify improvements for HEDIS outcomes
Review clinician capacity and productivity, partnering with Site Medical Directors on countermeasures when needed
Sign DME orders, death certificates, and other orders on behalf of APPs as regulated by state law
Perform routine panel reviews with APPs and assist with EMR efficiency
Provide patient care for assigned panel and high-risk or complex patients as needed, including cognitive assessments
Lead clinical education on accurate diagnosis, documentation, and chronic condition management
Handle discipline actions and performance reviews for direct reports
Support service inquiry resolution and patient complaint handling for the pod
Assist with compliance and HR resolutions as assigned
Requirements
Bachelor’s degree
Doctor of Medicine or Osteopathy degree from an accredited program
Completion of an accredited residency program
Unrestricted medical license
Unrestricted DEA registration
Active CPR certification
Valid driver’s license with good driving record
Board certification required for all hires or promotions on or after January 1, 2026
At least 3 years of experience as a physician collaborating with advanced practice providers
Experience in geriatrics or with complex patients
