Nurse Practitioner (NP)

The Nurse Practitioner role is with HarmonyCares, a national value-based provider of in-home primary care services for patients with complex healthcare needs. The position is based in Atlanta, Georgia, and operates within a team-based, physician-driven model serving high-acuity patients in their homes on a Monday-through-Friday schedule. HarmonyCares maintains practices across 14 states with more than 200 primary care providers.

Responsibilities

• Deliver primary care to high-acuity patients in their homes Monday through Friday
• Focus on addressing individual patient needs without emphasis on visit volume
• Apply experience in home health, geriatrics, hospice, and primary care settings

Requirements

• Active Nurse Practitioner license in Georgia
• Federal DEA license or eligibility
• Board certification
• Active CPR/BLS certification
• Valid driver’s license and good driving record
• Geriatric training or experience preferred
