Job Description

The Physician Leader, Primary Care role at HarmonyCares provides clinical oversight for 4-5 advance practice providers while delivering direct patient care to a panel of 140 patients in a home-based primary care model. The full-time position in Virginia follows a Monday through Friday schedule with no weekends or evenings and divides the week evenly between administrative oversight and hands-on visits for frail Medicare patients with chronic conditions. HarmonyCares has delivered in-home value-based care for more than 30 years across 14 states as part of an ACO REACH participant.

Responsibilities

  • Supervise assigned advance practice providers and physicians as the collaborating physician while ensuring the total number of collaborative practice agreements stays within state limits
  • Conduct at least one ride-along observation with each collaborative advance practice provider per quarter
  • Provide complex patient visits as needed for advance practice providers under supervision
  • Collaborate with community health workers, medical social workers, nurse navigators, care managers, and patient care coordinators on pod operations
  • Develop advance practice provider clinical skills through evidenced-based care training and professionalism guidance
  • Lead high-risk huddles and confirm documented plans of care that include all care management team members
  • Perform 10 chart audits per month or the number required by state regulations, whichever is greater
  • Review quality metrics and pod-level performance to identify improvements that support HEDIS outcomes
  • Monitor clinician capacity and productivity and partner with site medical directors to implement countermeasures when performance falls off track
  • Sign DME orders, death certificates, and other documents on behalf of advance practice providers as permitted by state regulation
  • Support advance practice providers with difficult patient or family conversations and serve as a resource for clinical questions
  • Conduct routine panel reviews with advance practice providers and assist with EMR efficiency
  • Deliver patient care for the assigned panel and high-risk or complex cases, including physician face-to-face requirements and cognitive assessments
  • Lead clinical education on accurate diagnosis, documentation, and chronic condition management
  • Handle discipline actions and performance reviews for direct reports and assist with compliance and HR resolutions
  • Partner with the market manager in a dyad structure and work with the regional medical director on care improvement opportunities

Requirements

  • Bachelor’s degree
  • Doctor of Medicine or Osteopathy
  • Active medical license
  • Active CPR certification
  • Valid driver’s license with a good driving record
  • Board certification preferred
  • One year of experience at HarmonyCares Medical Group preferred
