Job Description

HarmonyCares operates one of the nation’s largest home-based primary care practices and maintains affiliated companies delivering medical group, home health, and hospice services. The Community Liaison Marketing & Business Development role is based in Renton, Washington, and focuses on local market outreach to referral partners and eligible patients for enrollment in coordinated in-home care programs. The position involves both in-person and virtual engagement within an assigned territory and supports patient transitions from area facilities.

Responsibilities

  • Conduct market analysis of the competitive landscape to position HarmonyCares services to external referral partners
  • Educate community partners on the company’s mission and service offerings for high-quality personalized healthcare delivered in the home
  • Meet or exceed targets for in-person and virtual meetings with key decision makers
  • Explain eligibility criteria, enrollment processes, and program benefits to patients, caregivers, and legal representatives
  • Facilitate voluntary alignment with accountable care organizations or other CMS alternative payment models and collect required forms
  • Document outreach activities, scheduled meetings, and performance metrics in the CRM system against regional budget targets
  • Schedule and conduct face-to-face or telephone discussions with patients and caregivers regarding program eligibility
  • Answer inbound calls from patients, families, and facility staff concerning the company’s business units in the market
  • Triage patient concerns related to care needs, scheduling, and billing to the appropriate internal department
  • Oversee and record all outreach calls and external meetings
  • Comply with all company policies, applicable laws, marketing regulations, HIPAA privacy and security requirements, and beneficiary notification rules
  • Maintain regular communication with sales leaders and local business unit leadership
  • Perform additional duties as assigned

Requirements

  • Bachelor’s degree or four or more years of B2B sales experience
  • Two or more years of healthcare industry sales or marketing experience
  • Valid driver’s license and acceptable driving record
  • Proficiency with Microsoft Office applications including Word, Excel, and Outlook
  • Ability to travel throughout the assigned territory and attend required sales meetings
